Home care in Massachusetts area

My father has stage four glioblastoma and has completed radiation and chemo therapy. Our challenge now is neither my father nor the local family is up to the task of monitoring medication, coaching him to make sure he eat/drinks and gets the right amount of exercise. Neither are they observant enough to notice changes and aggressively get the right medical care. I live far away and, while I visit frequently, know I cannot substitute for someone who is physically there daily, or at least frequently. 

Do any of you know of professional assistance that would have someone make frequent calls to help with these types of issues? Financially the family can make it happen; the question is finding someone who can come frequently and provide the attentive help we need to get through the next few months until he is more stable. 

Any guidance would be very welcome!

For my mom they did a great work, that's why I am telling you about them.
